As you know, you and your classmates identified interesting
questions that you would like answered about the student population.
Your instructor has taken a sample of these questions and created the following
data form. You are now requested to complete the resulting
data collection form. The data that are collected will be used only
by you and your classmates to complete your final project for this course.

In completing the form, you will remain anonymous and confidential.
That is, no identifying names or ID numbers will be attached to your data.

You may choose not to respond to some questions.
This may happen if you feel uncomfortable with the content of the question
or if the question does not apply to you. In either case, just
leave the question unanswered and proceed to the next question.

The form will take approximately 5 minutes to complete.
You should only begin completing the form if you have enough time to
complete the form in its entirety. Once you have begun completing
the form, there is no way to save your data so that you can resume again
later.

In completing the form, you will encounter three different
types of questions:

Radio button questions:

By default, all radio button questions have the "No response" option
selected. To respond to the question, use your left mouse button
to select the appropriate answer. If you do not want to respond to
the question, select the default "No response" option. Feel free to practice
on this radio button question now.

Menu option questions:
What is your height in inches?

To respond to the question, use your left mouse button to click on the
down arrow. The menu of options will appear. Then, drag your
mouse to the appropriate answer, and click on your left mouse button.
If you do not want to respond to the question, select the default "blank"
option. Feel free to practice on this menu option question now.

Fill-in-the-blank questions:

To respond to the question, use your left mouse button to click on and
activate the empty box. Once the cursor is in the box, type your answer
in the box. If you do not want to respond to the question, just leave the
answer box empty, and proceed to the next question. Feel free to practice
on this fill-in the blank question now.

along with the left mouse button to answer the questions. When you are
ready to complete the data collection form, just click on the button
